DESCRIPTION OF INDIVIDUAL: Individual is a 44-year-old man who lives with his mother in Mentor. He wants to get out of the home and socialize as well as access the community. He is also interested in exploring vocational skills. Individual uses a catheter 3x day – does independently. Would not be using at program.

TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: Spina Bifida and Congenital Hydrocephalus
HOUSING: Lives at home in Mentor near 615.
