DESCRIPTION OF REQUESTED SERVICES: Client is interested in attending a day program five days a week. Client would need transportation to and from their program. Client is able to walk on her own but sometimes needs assistance (a hand) when walking down steps or on uneven surfaces. Client will need total assistance in meal prep and at times will need physical assistance using the restroom. Client is interested in a day program that has a lot of activities and goes out into the community. Day program would need to be ok with the client going to see her dad out of state several times a year.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Client is interested in an Adult Day Program.
FUNDING SOURCE: Level One Waiver.

DESCRIPTION OF INDIVIDUAL: Client’s family is very important to her. Client lives with her mom but goes and stays with her dad out of state several times a year. Client is able to walk on her own but there are times when she will need a hand getting down steps or on uneven surfaces. There are times that the client wears depends and times when she will need assistance using the restroom. Client requires full assistance with meal prep and ADLS. Client does like to help though. Client is afraid of strange dogs. When client gets mad or upset, she will whine. Client will also sometimes push things away with her arms. If client does not want to leave somewhere she will sit down. Client is nonverbal but is able to communicate with gestures to people that know her. Client loves Veggie Tale books and movies. Clients Veggie Tale books are very important to her. Client likes to be read to. Client needs time to process her thoughts and to be given choices. Client is very active in extra (sports) activities. Client would like a day program that is very active and goes out into the community a lot.
PROVIDER REQUIREMENTS: Client takes medications to treat Tourette’s, menses regularity, and Hypothyroidism. Client needs total assistance taking her medications.
TRANSPORTATION NEEDS: Non modified vehicle.
DIAGNOSES: Client is diagnosed with Down’s Syndrome, Hypothyroidism, OCD, and Tourette’s.
HOUSING: Client lives with her mom but will visit her dad out of state several times a year.
DAY PROGRAM/EMPLOYMENT: Client is not interested in employment.
