DESCRIPTION of NEEDED SERVICES: The individual is a 48-year-old male who lives in Wickliffe. Looking for a nurse to cover for a nurse that is taking some time off from the 22nd to the 27th of October. PDN services from 10a.m. to 6p.m up to 12-hour days for 7 days a week could be authorized. The family is looking for weekends and Mondays. The individual is diagnosed with intellectual disability NOS, Grand Mal Status, Impulse Control, CP NOS, Esophageal Reflux, Optic Atrophy, Constipation, Incontinence, Blindness in left eye, Seizures with apnea Chris has grand-mal seizures with apnea.

PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.

HOUSING: This individual lives with her mother and small dog, Ainee in Wickliffe Ohio
DAY PROGRAM: Individual is not currently enrolled in a day program.
SUPPORTS NEEDED: This individual is looking for assistance with seizure management, bathing, changing clothes, medication administration, and meals.
