DESCRIPTION OF REQUESTED SERVICES: This individual’s guardian is seeking assistance with HPC staffing in the home to assist with developing consistent hygiene routines, daily living skills, and provide supervision and redirection from potential risk behaviors. Family is seeking HPC 3-5 days a week, days to be determined. flexible regarding hours.
SEEKING AGENCY OR INDEPENDENT PROVIDER: open to either agency or independent provider.

DESCRIPTION of INDIVIDUAL: This individual is an 8-year-old diagnosed with Autism who lives with her family in Painesville, OH. She requires visual supervision at home, and Close and Constant Supervision in the community. She is partially verbal and can communicate in short phrases although she communicates her wants largely through gestures, or by bringing a person to an area or an item to a person. She has familiar stims that can help identify her mood, especially when happy or mischievous. If she does not want to do something, she will protest through yelling, crying, or self-injury. She is very creative and curious and excels in reasoning tasks. She learns best through visual instruction and modeling. She does not demonstrate an understanding of danger in any situation. She will touch hot objects, consume harmful chemicals, walk into traffic, etcetera. She is an elopement risk, and able to figure out complicated locks and other obstacles that may prevent or inhibit elopement. She may engage in property destruction if bored. She is rarely aggressive, though she may defend her food or lash out if redirected from self-injury. She has many effective strategies for self-regulation and learns new tasks quickly. She is very personable and friendly with her family and siblings. This individual loves to go swimming and be outdoors.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Autism Spectrum Disorder
HOUSING: resides with family in Painesville.
DAY PROGRAM/EMPLOYMENT: Individual is of School-age.
