DESCRIPTION OF REQUESTED SERVICES: 40-year-old male individual in need of one week of Residential Respite each month (up to 90 days per span). The individual will need Total Assistance for all ADLs and medication administration. He requires Auditory Supervision while he is in the respite setting, with 8 hours of OSOC, and Visual Supervision while he is in the community.
The individual would prefer if the respite provider could also provide morning NMT during the week of respite, but his ADS provider is able to provide NMT if this isn’t an option. He doesn’t require a modified vehicle.

DESCRIPTION OF INDIVIDUAL: Individual is a 40-year-old male. He is generally happy and smiling every day, gentle, and compliant with any directions/requests presented. He does not have behavior issues and can share a space with other individuals/animals peacefully. He loves to people-watch and can spend hours looking out the window or sitting in the mall, bowling alley, or other public location to watch others as they pass by. He is fascinated by construction vehicles, trucks, and buses and will watch the traffic out the window. He loves to go for a ride, even if it’s just to run a quick errand.
PROVIDER REQUIREMENTS: Medication administration certification (Category 1 – oral, topical only)
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Cerebral Palsy – Spastic Quadriplegia type; Autism; Profound Intellectual Disability (FSIQ<20)
HOUSING: Residential respite setting
DAY PROGRAM/EMPLOYMENT: Currently attends ADS programming Monday-Friday in Mentor.
