Intake and Eligibility

Any Lake County resident with developmental disabilities may be assessed for eligibility for County Board services.

  • 0-3 must have one documented delay or developmental disability diagnosis
  • 3-6 must have two documented delays or a developmental disability diagnosis
  • 6-15 must have a documented developmental disability diagnosis and at least 3 substantial functional limitations out of 6 life skill areas assessed using the Children’s Ohio Eligibility Determination Instrument (COEDI)
  • 16+  must have a documented developmental disability diagnosis and at least 3 substantial functional limitations out of 7 life skill areas assessed using the Ohio Eligibility Determination Instrument (OEDI)

Intermediate Care Residential Services

The Residential Program (ARC & JCDC/Respite) provides 24/7 active treatment and nursing care for individuals with developmental disabilities in a campus-like setting.

Medicaid Home and Community Based (HCBS) Waiver Services
