Intake and Eligibility

Any Lake County resident with developmental disabilities may be assessed for eligibility for County Board services.

  • 0-3 must have one documented delay or developmental disability diagnosis
  • 3-6 must have two documented delays or a developmental disability diagnosis
  • 6-15 must have a documented developmental disability diagnosis and at least 3 substantial functional limitations out of 6 life skill areas assessed using the Children’s Ohio Eligibility Determination Instrument (COEDI)
  • 16+  must have a documented developmental disability diagnosis and at least 3 substantial functional limitations out of 7 life skill areas assessed using the Ohio Eligibility Determination Instrument (OEDI)

Intermediate Care Facility

The Residential Program (ARC & JCDC/Respite) provides 24/7 active treatment and nursing care for individuals with developmental disabilities in a campus-like setting. The Residential Program serves over 50 permanent individuals. Our goal is to support individuals with the skills and abilities necessary to attain independence in Adult Daily Living skills and grow as individuals in becoming valued members of their chosen communities.
