DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 35-year-old male with a good sense of humor. He resides with his parents in Wickliffe and works at Telshe Yeshiva. Male Staff only!!
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Downs Syndrome
HOUSING: He resides with his family in Wickliffe.
DAY PROGRAM: Community Employed
SUPPORTS NEEDED: Transportation, Community Membership, Safety, Health, exercise and socialization.
