DESCRIPTION of NEEDED SERVICES: 54-year-old male individual seeking an HPC provider around 4 hours per week. Looking to have an increase in community involvement. He is home from program around 2:30PM Monday-Friday and would be open to using HPC hours after program or on the weekend.

DESCRIPTION of INDIVIDUAL: Friendly and kind individual who likes to know he is doing a good job and people are proud of him. He loves his animals and family. He loves police departments, fire departments, trains and all things related. He enjoys purchasing trucks to play with. He likes to spend the money he earns. He loves Dr. Pepper, going out to eat, going to parks, and taking walks. He enjoys activities with other peers where he can socialize. He often needs support to recognize appropriate topics of conversation when in the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Autism
HOUSING: Living with his mother in Mentor.
DAY PROGRAM: Currently attends Brighter Horizons Monday-Friday.
SUPPORTS NEEDED: This individual is able to do most ADLs independently. He does well with verbal prompting. He will need support to stay on task, reduce distractions, and appropriately socialize. He will also need transportation to and from planned community outings.
