DESCRIPTION of NEEDED SERVICES: The individual is a 48-year-old male who lives in Wickliffe. The individual is looking for bathroom renovation to meet his needs. The PT eval indicated that the height of the tub is too high and it has no grab bars. The PT eval recommends a non-skid surface approx. flooring, an elevated toilet with a swing up grab bar next to the toilet, a rounded vanity top to prevent head injuries, and a recessed medicine cabinet. The individual is diagnosed with intellectual disability NOS, Grand Mal Status, Impulse Control, CP NOS, Esophageal Reflux, Optic Atrophy, Constipation, Incontinence, Blindness in left eye, and grand-mal seizures with apnea.
