DESCRIPTION of NEEDED SERVICES: The individual is a 48-year-old male who lives in Wickliffe. The individual is looking for a tub with shower renovation to meet his needs. The PT eval indicated that the height of the tub is too high and it has no grab bars. The PT eval recommends a non-skid surface approx. 14”-16”, no protruding shelves, vertical grab bar at the entrance of the shower, an additional grab bar on the inside wall of the sub/ shower for safety, switch release for the tub, and a handheld shower The individual is diagnosed with intellectual disability NOS, Grand Mal Status, Impulse Control, CP NOS, Esophageal Reflux, Optic Atrophy, Constipation, Incontinence, Blindness in left eye, and grand-mal seizures with apnea.
