DESCRIPTION OF REQUESTED SERVICES: Individual is looking for residential respite services for 1-2 Saturdays a month. The hours are flexible but averaging about 4-5 hours each outing.

DESCRIPTION OF INDIVIDUAL: Individual is a 22-year-old male living in Mentor with his mom and shared living provider. Individual has a behavioral add on. Individual may become frustrated when he needs help or does not like something. He does enjoy being active. He enjoys cooking, being outside, collecting sea glass, and loves sports cards. He loves sports (playing basketball).
PROVIDER REQUIREMENTS: Residential respite
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism, Anxiety, and OCD
HOUSING: Individual lives with his family in Mentor
DAY PROGRAM/EMPLOYMENT: Attending Hilltop Supported Living
