DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC staff from 10:00a-10:00p 7 days/week. This individual requires total assistance with all ADL’s, feeding, medical treatments and transfers. This individual’s mother provides natural support and would provide all necessary training outside of delegated nursing tasks.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Agency or Independent providers who are willing and able to provide services as written in individual’s ISP.

DESCRIPTION OF INDIVIDUAL: The individual is a 35-year-old male who lives in his family home in Mentor, OH with his mother. His main diagnoses include Spastic quadriplegia, cerebral palsy, GERD, acid reflux, outlet dysfunction, constipation, small bowel tube feeding, s/p percutaneous endoscopic gastrostomy (peg) placement, asthma, anxiety, thrombocytopenia, urinary retention issues, malnutrition of mild degree, dislocated hip (left), recurrent urinary tract infections and diarrhea, normal pressure hydrocephalus, cervical dystonia and neurogenic bladder. He has a history of developing pressure sores if in one position for too long. The individual also uses a percussion vest once a day to help break up mucus in his lungs to be suctioned out. All feeds and medications are administered via J-Tube, and all drainage is to be done via G-Tube. This individual is non-verbal and communicates through body language. He enjoys when people sit and talk with him, listening to music or watching tv with others.
PROVIDER REQUIREMENTS: Provider must have Med. Certification 2 with nurse delegation.
TRANSPORTATION NEEDS: Requires modified vehicle, but guardian has a modified van that can be used for any transportation needs.
