DESCRIPTION OF INDIVIDUAL: Individual is a 42-year-old female. Individual enjoys making money and using her money to spend on things she enjoys. She enjoys having nice nails. She also has two small dogs and enjoys caring for them. Individual needs encouragement to make healthy choices for her diabetes. She needs support to learn expectations for community employment (ex: work appropriate clothing, good hygiene, caring for health needs to reduce absences). She receives prompts to check her work for thoroughness. Positive feedback helps her.
PROVIDER REQUIREMENTS: GES or VH Certification
TRANSPORTATION NEEDS: Will receive NMT services from current provider.
DIAGNOSES: frequent UTIs, diabetes, lactose intolerance, has migraine headaches, restrictive lung disease. She has been diagnosed with Schizoaffective Disorder, Borderline Personality Disorder, developmental delays, bipolar disorder, Depression, and Arthritis.
HOUSING: Lives in Willowick in group home.
DAY PROGRAM/EMPLOYMENT: Looking for new GES or VH provider.
