DESCRIPTION OF REQUESTED SERVICES: HPC services provided in or out of the home are being sought by the individual. Specifically, HPC is needed 6:30 am – 8:30 am, Monday-Friday to help individual with her morning routine; ensure she is dressed and eats breakfast; has her lunch packed and belongings ready to take with her to ADS; and alert her when her NMT vehicle arrives to transport her to ADS. Additional HPC hours may be sought if the individual’s ADS program is canceled/closed for the day. She can walk independently but occasionally requests someone’s hand for assistance when navigating curbs/steps or walking across uneven terrain. The individual will need total assistance in meal prep. She requires auditory supervision in the home with visual check-ins, and visual supervision while in the community.

DESCRIPTION OF INDIVIDUAL: Individual has limited verbal skills and typically communicates through gestures and vocalizations. The individual loves to have Veggie Tale books read to her and enjoys watching television. She needs time to process the choices presented to her and to respond accordingly. The individual participates in Miracle League and Metropark Adaptive Sports. She likes to be around people and watch people. She can be hesitant at first to join in activities, but with time, she will participate with assistance from her support person.
PROVIDER REQUIREMENTS: Family would prefer a female provider who is non-smoking.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Individual is diagnosed with Down’s Syndrome, Hypothyroidism, OCD, Tourette’s, ASD, and ADHD.
HOUSING: Individual lives with her mom in Perry and visits her dad, who lives out-of-state, several times a year.
DAY PROGRAM/EMPLOYMENT: Attends a day program in Mentor.
