DESCRIPTION OF REQUESTED SERVICES: NMT services in a non-modified vehicle to transport the individual to/from her home in Perry and her ADS Program in Mentor, Monday-Friday. NMT pick-up MUST be before 8:00 am due to her caregiver’s work schedule.  She can walk independently but occasionally requests someone’s hand for assistance when navigating curbs/steps or walking across uneven terrain.

FUNDING SOURCE: Level One Waiver.

DEADLINE TO RESPOND: To be considered for an interview, please submit a response through the RFP portal. Be sure to include the referral (RFP) number, and your contact information in your response.  RFPs are active in the portal for two weeks from date of posting.

DESCRIPTION OF INDIVIDUAL:  Individual has limited verbal skills and typically communicates through gestures and vocalizations. The individual loves to have Veggie Tale books read to her and enjoys watching television. She needs time to process the choices presented to her and to respond accordingly. The individual participates in Miracle League and Metropark Adaptive Sports. She likes to be around people and watch people. She can be hesitant at first to join in activities, but with time, she will participate with assistance from her support person.

PROVIDER REQUIREMENTS: Family prefers a driver who doesn’t smoke.

TRANSPORTATION NEEDS: Non-modified vehicle.

DIAGNOSES: Individual is diagnosed with Down’s Syndrome, Hypothyroidism, OCD, Tourette’s, ASD, and ADHD.

HOUSING: Individual lives with her mom in Perry and visits her dad, who lives out-of-state, several times a year.

DAY PROGRAM/EMPLOYMENT: Attends a day program in Mentor.
