DIAGNOSES: Mild intellectual disability, cerebral palsy, scoliosis/kyphosis, hydrocele, H/O bronchial asthma, amblyopia OD, high blood pressure, astigmatism, hypertension.
HOUSING: Apartment
DAY PROGRAM/EMPLOYMENT: Individual works M-Th from 11:30-2:00 in Mentor.
