DESCRIPTION OF REQUESTED SERVICES: Looking for an HPC provider to provide services to a 16-year-old male in Willowick, Ohio. Individuals want to focus on community outings, two evenings a week for three to four hours per shift. *Non-smoker.

DESCRIPTION OF INDIVIDUAL: Individual is a sixteen-year-old male. He enjoys swimming, working out, video games, and reading books. Individual wants to work on learning adult life skills-cooking, making grocery lists, making purchases, ect. Individual is kind and gets along well with others.
PROVIDER REQUIREMENTS: HPC and HPC Transportation. Needs a provider who is a non-smoker.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Individuals’ diagnosis includes: autism, chronic lung disease, asthma, short-term memory loss.
HOUSING: In the community and Willowick, Ohio.
