DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an HPC provider at home and for engagement in community activities. This individual needs physical to total assistance with all hygiene-related routines. Individual is looking for two Saturdays per month from 4PM-11PM with the occasional need for overnight support or additional Saturdays but this will be infrequent.

DESCRIPTION OF INDIVIDUAL: This individual loves going out to eat, going to get pop, car rides, having her nails done and listening to music. This individual is mostly non-verbal but does use signs and a few single words or sounds to communicate. Individual needs visual supervision in the home and especially in the community as she will wander off and does not have any safety skills. Individual does best with a schedule or social stories and a timer to help with transitions. If frustrated, this individual may hit, pinch or bite herself or others. This individual will throw objects out of frustration and has also flipped tables.
PROVIDER REQUIREMENTS: HPC and HPC transportation.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: OCD, Anxiety, Seizures, Episodic Ataxia Type 2
HOUSING: Individual lives in Painesville with family.
DAY PROGRAM/EMPLOYMENT: Not currently in a day program
