DESCRIPTION OF REQUESTED SERVICES: Individual seeking Adult Day Support. Individual uses a wheelchair and needs physical assistance with transfers and toileting but can maneuver the chair himself. Individual would need assistance with medicine and nutrition via G-tube, and trach care (nursing). He may need assistance from a nurse cleaning his trach canula.

DESCRIPTION OF INDIVIDUAL: Individual is 29 years old and resides in Kirtland. He uses a wheelchair for mobility and requires physical assistance with transfers, and total assistance with most other daily tasks. He communicates with gestures and vocalizations, largely home signs. Family would assist in understanding the multimodal forms of communication.
PROVIDER REQUIREMENTS: Med cert 1 and 2 needed, nurse needed for intermittent trach care

DAY PROGRAM/EMPLOYMENT: Seeking
