DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an Adult Day Support program that she can attend 2-3 days a week. She is looking to be more active in her community and make friends.

DESCRIPTION OF INDIVIDUAL: 54-year-old female who is soft spoken, kind and friendly and values her independence. She enjoys playing games on her phone and having her daily coffee. She needs encouragement from provider to follow her diabetic diet, drink more water, and assistance managing her finances.
PROVIDER REQUIREMENTS: She requires Med Cert 3 staff to monitor her when she is administering her insulin daily.
TRANSPORTATION NEEDS: She would like transportation by provider, but she is capable of arranging another NMT service herself if needed.
DIAGNOSES: Type 1 Diabetes, Anxiety, Schizophrenia, Depression, Osteoarthritis, Rheumatoid Arthritis, Esophageal Constriction, hypertrophic obstructive cardiomyopathy, coronary artery disease, GERD, asthma, hypertension, hypercalcemia, osteoporosis, non-intractable Epilepsy without status epilepticus.
HOUSING: She lives by herself in a group home in Painesville
DAY PROGRAM/EMPLOYMENT: Looking for ADS program 2-3x a week.
