DESCRIPTION OF REQUESTED SERVICES: Individual is looking for NMT to and from their day program. This individual lives in Willoughby and attends a day program in Painesville on Mondays, Wednesdays and Fridays.

DESCRIPTION OF INDIVIDUAL: This individual enjoys riding to and from the day program, especially if she can sit in the front seat. Individual has general supervision in the home and visual supervision in the community. This individual does not like to have too many demands placed onto her and needs clear, concise directions as she becomes easily distracted. Individual does not like loud noises. This individual does best in a smaller setting with few people and may become agitated and yell or cover ears/pick at fingers when there is too much noise.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Autism, ADHD, OCD, and Moderate ID.
HOUSING: Individual lives in Willoughby with dad and grandma
DAY PROGRAM/EMPLOYMENT: Individual attends a day program on Mondays, Wednesdays and Fridays.
