DESCRIPTION OF REQUESTED SERVICES:
Looking for an HPC/NMT provider Mon-Fri in the mornings. Client needs some physical assistance with tying shoes and zipping coats. Needs assistance with making sure the house door is locked. Individual currently takes Lake Tran in the AM to the day program in Mentor, but NMT is possible if willing to drive client and drop off at the program. If not willing or able to provide NMT, HPC to provide assistance with waiting for and getting onto the Lake Tran bus.

DESCRIPTION OF INDIVIDUAL:
Individual is a female in her 20s who lives at home with her Dad. Dad has to be at work early in the AM and needs someone to help her get out the door safety and with her things. Individual does not take meds. She is verbal and friendly. She likes to sing and have fun. She likes to listen to music.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Individual DX Down Syndrome
HOUSING: Individual lives with Dad in Mentor.
DAY PROGRAM/EMPLOYMENT: Individual attends a day program M-F in Mentor.
