DESCRIPTION of NEEDED SERVICES: Individual is a 43-year-old male. He would like to locate an ADS/Voc Hab program to attend two times a week.

DESCRIPTION of INDIVIDUAL: The individual is fun to be around and likes to socialize, help around the house, has a great sense of humor, is compassionate and easy going. He participates in a bowling league, enjoys outings in the community (mall walking), listening to music, collecting t-shirts, maps and license plates. He would need visual supervision in the community; he is in the process of getting hearing aid.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild I/D and Downs Syndrome
HOUSING: Individual lives with his family in a home in Mentor.
DAY PROGRAM: N/A
SUPPORTS NEEDED: Nick would like to participate in a voc/hab day program where he can meet others and socialize while continue to develop his work skills.
