DESCRIPTION OF REQUESTED SERVICES: Looking for HPC/HPC-T services. The individual lives with his elderly mother and father. He is seeking services on Saturdays and Sundays to provide care while his father works. Typical hours needed is 8-4pm but can be somewhat flexible when determining schedule. Schedule starts November 29 & 30th.

DESCRIPTION OF INDIVIDUAL: A friendly, caring 67-year-old male that can carry on conversations but may repeat back what someone is saying as he may not understand or need time to process. He does enjoy playing games, Snoopy, his beagle plushies, cats, and taking breaks in his power recliner at home. He also enjoys going out in the community and socializing with others. He does have frequent health issues, which may slow him down physically at times and uses a walker to assist him first thing in the morning. Needs assistance with most ADLs. This individual is safest with auditory supervision within the home and visual supervision in the community.
PROVIDER REQUIREMENTS: HPC/HPC-T certified
TRANSPORTATION NEEDS: Non- Modified vehicle
DIAGNOSES: Moderate ID, Impulse control D/O, High Blood Pressure, Urinary retention.
HOUSING: Lives in South Mentor Home with his parents
DAY PROGRAM/EMPLOYMENT: Looks forward to attending ADS program 2x weekly when his medical issues permit him to go.
