DESCRIPTION of NEEDED SERVICES: Individual is seeking a congregate setting to which to move due to recent life changes. She is independent in many routine tasks and may need verbal reminders to complete cleaning, but is typically able to do it with minimal help. A home with minimal stairs is preferred.

DESCRIPTION of INDIVIDUAL: Individual is verbal and able to express herself and what she needs, for the most part. She is 51 and currently lives alone in the family home with family/staff supervision.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild ID, ulcers, hypothyroid, high blood pressure, Moyamoya disease, penicillin allergy
HOUSING: Lives in Wickliffe
DAY PROGRAM: M-F
SUPPORTS NEEDED: HPC – Congregate
