A background check is required for all persons seeking certification as an independent or agency provider (CEO). All employees of an agency provider must also receive a background check. Background checks must be completed prior to hire and occur at the frequency defined in rule.

For certification and recertification purposes, background checks must be sent directly to DODD:

Bureau of Criminal Identification and Investigation Criminal Records Check (BCII) is required for all persons seeking certification / employment with an agency

  • For agency CEO / Independent Provider use code 5123.169

Bureau of Criminal Identification and Investigation Federal Bureau of Investigation Criminal Records Check (FBI) is required for all persons seeking certification / employment with an agency who have not been a resident of Ohio for the five-year period immediately prior to the date the criminal records check is requested

  • For agency CEO / Independent Provider use code 5126.28

Background checks can be obtained at a number of locations throughout Lake County. You can use this website to search for a location near you. It is recommended to call ahead to ensure the location is currently providing background checks and to see if you are required to schedule an appointment or bring any additional information.
