DESCRIPTION of NEEDED SERVICES: Money management/rep payee services needed for individual. Individual receives income and needs assistance managing money, paying all bills and maintaining benefits with social security and JFS.

DESCRIPTION of INDIVIDUAL: Individual is currently employed through her voc hab program. She also receives social security income. She participates in community integration and has a goal of being more active in the community. She enjoys her collection of hats, going to yard sales, and buying jewelry.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Cerebral Palsy, hypothyroid, depression, anxiety, onychomycosis, high cholesterol, frequent UTIs, constipation, dry ears/wax build up, constipation.
HOUSING: Lives with a roommate in Mentor, OH
