DESCRIPTION of NEEDED SERVICES: 60 year old female individual searching for wheelchair accessible congregate waiver home. She could do well with male or female roommates. Will need peg tube feedings 4x per day.

DESCRIPTION of INDIVIDUAL: Individual is motivated to move. She voiced the desire to have more independence and access the community on a more regular basis. She can be described as a social butterfly who loves shopping. She is a strong advocate for herself and her needs/wishes. She has goals to attend church, concerts, and on vacations. She has strong connections with family and friends. She wants to work in a restaurant setting. This individual has communication barriers, but can be redirected to use a letter board & her iPad to explain her needs. Her current ICF did voice a history of behaviors and the team is working to gather more detailed information.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Major Depressive disorder, Anxiety, Dysthymic, Mild ID, Borderline Personality, Oppositional Defiant, Cerebral Palsy, Osteoporosis, Pharvngo, neurogenic bladder, allergic rhinitis, history decubitus ulcer, aphasia, history of UTI’s, recent memory loss & Parkinism.
HOUSING: Looking for housing in Painesville, Mentor, or Willoughby. Currently living in an ICF (since 2013)
DAY PROGRAM: Willing to look at day programs once she moves.
SUPPORTS NEEDED: This individual is in a wheelchair, needs support to be tube fed 4x per day as well as total support for medication. This client will require physical to total assist with all hygiene care, and ADL’s. She requires total support to change her attends every two hours. She requires total support in the shower, and has a shower gurney. She has a high low bed with quarter side rails.
