DESCRIPTION of NEEDED SERVICES: Individual is looking for an ADS program after graduating High School in April 2025. Individual would likely do a slow transition up to 5 days a week. Open to discuss days/times. Individual is in school during the week.

DESCRIPTION of INDIVIDUAL: Individual is a 21 year old male with Autism Spectrum Disorder. Individual attends Mentor CARES. Individual needs 24/7 support.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, ADHD, Anxiety, Epilepsy, boils, Tic Disorder, OCD and low muscle tone.
HOUSING: He lives with his mother and father in Mentor, close to the Great Lakes Mall.
DAY PROGRAM: Attends CARES
SUPPORTS NEEDED: Recreational activity, supervision, behavior support, meal preparation.
