DESCRIPTION of NEEDED SERVICES: 53 year old male individual seeking an HPC provider for 4-8 hours per week. Looking to have an increase in community involvement.

DESCRIPTION of INDIVIDUAL: Friends and kind individual who likes to know he is doing a good job and people are proud of him. He loves his animals and family. He loves police departments, fire departments, trains and all things related. He enjoys purchasing trucks to play with. He likes to spend the money he earns. He loves Dr. Pepper, going out to eat, going to parks, and taking walks.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Mild Intellectual Disability, Autism
HOUSING: Living with his mother in Mentor.
DAY PROGRAM:
SUPPORTS NEEDED: This individual is able to do most ADL’s independently. He does well with verbal prompting. He will need support to stay on task and reduce distractions. He will also need transportation to and from planned community outings.
