DESCRIPTION of NEEDED SERVICES: This individual is a 52 year old female residing in her family home in Mentor. She is seeking HPC services to go into the community one time per week for 2-3 hours to participate in preferred leisure activities. Days and times are flexible.

DESCRIPTION of INDIVIDUAL: The individual is able to communicate verbally but it is difficult for those who do not know her well to understand due to her hearing loss and speech impediment. She requires monitoring for low Oxygen levels and is able to ambulate for short distances and prefers to hold someone’s hand if she is navigating uneven surfaces or feels nervous. Individual has a great sense of humor and enjoys going out for coffee, drawing, listening to old music and watching old movies.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Down syndrome, Moderate ID, Bilateral Hearing Loss, GERD, Alopecia, Obesity.
HOUSING: Lives in family home with her brother.
DAY PROGRAM:
SUPPORTS NEEDED: Assistance with communication with others, provide encouragement to expand her interest in varied activities, supports needed to monitor Oxygen levels.
