DESCRIPTION of NEEDED SERVICES: This individual is 25 year old female who is looking to explore a new day program.

DESCRIPTION of INDIVIDUAL: She is verbal and will tell you her likes and dislikes. She enjoys learning and enjoys the school setting. She likes to do sports, bowling, going to the zoo and putt-putt.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Down’s Syndrome, Hypothyroid, Migraines ( Chiari-Malformation) Nearsighted, Alopecia, TMJ, Mild hearing loss, Reactive airway Seasonal allergies and allergic to cats
HOUSING: She lives in Mentor with her mom and brothers.
DAY PROGRAM: Searching for a day program.
SUPPORTS NEEDED: She requires assistance with uneven surfaces, steps, and curbs. She needs assistance with meal prep (cutting food) and meal planning. She rides in a non-modified vehicle.
