DESCRIPTION of NEEDED SERVICES: These individuals are seeking a new Residential Provider for a new congregate living environment. They are largely independent, but need reminders for routine cleaning, hygiene, and taking meds. Some routine cleaning tasks may required verbal to physical assistance to complete. The individuals also need assistance with transportation to the community and to appointments. They currently receive Remote Supports with Paid Backup overnight and would prefer a provider able to provide that service.

DESCRIPTION of INDIVIDUAL: The first individual is a 26-year-old verbal male diagnosed with Autism, the second is a 29 year old male diagnosed with bipolar disorder, and the third is a 31 year old male diagnosed with Autism and renal failure. It is very important for the third individual to have consistent transportation to his medical appointments due to ongoing medical needs, but he is otherwise very independent. All individuals are ambulatory, verbal, and capable of independence with most tasks. They are all avid gamers.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: 1) Autism, bi-polar, mitochondrial metabolic disorder, adjustment disorder, proximal SVT, pervasive disorder, hyperthyroid, mixed emotions with disturbances of conduct, vesicular anomaly and acid reflux; 2) Schizo-Effective Bipolar Disorder, Developmental Delay, PTSD; 3) Mild ID, Autism, renal failure, other
HOUSING: Individuals live in a congregate setting, seeking a new provider.
DAY PROGRAM: N/A; individual 3 is employed in the community
