DESCRIPTION of NEEDED SERVICES: HPC services and HPC transportation services with no alone time and OSOC during sleep hours. General supervision in the home and visual in the community.

DESCRIPTION of INDIVIDUAL: Individual is a 63-year old male who enjoys watching television, doing crafts, and making his own daily decisions. He is verbal, speaks English, and can communicate his wants, needs, and feelings. Individual enjoys participating in community outings and activities, is social, and spending time alone in his room. He is independently mobile and needs no assistive devices, but can be unsteady on uneven surfaces.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Downs Syndrome and arthritis
HOUSING: Residential home in Madison with three male housemates.
DAY PROGRAM: Voc Hab in Painesville with NMT provided
SUPPORTS NEEDED: Verbal to physical assistance with personal hygiene tasks, household chores, laundry, meal planning, meal preparation, and shopping for groceries and personal items. Assistance finding and participating in community activities with HPC transportation. Individual has no healthcare concerns needing continuous monitoring or attention, however, does need assistance with all aspects of healthcare appointments.
