DESCRIPTION of NEEDED SERVICES: Individual is seeking an HPC provider to assist with hygiene and over tasks at home, possibly in the community. The guardian is hoping to find assistance 2-3x/week with these needs. In particular, he would need assistance with shaving, as well as gestural to physical assistance with shower.

DESCRIPTION of INDIVIDUAL: Individual is a 30-year-old male living in Madison, OH. He is diagnosed with Autism. He walks with a slow, unsteady gait due to a fear of falling over, and would need assistance with ambulation in any possibly challenging terrain. He is partially verbal, but has recently preferred gestural communication due to a surgery making speech difficult.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, Seizure history, left foot drop
HOUSING: Lives with family in Madison
DAY PROGRAM: 5 days/week
SUPPORTS NEEDED: Assistance with hygiene routines, possibly community access. Assistance with communication and ambulation.
