DESCRIPTION of NEEDED SERVICES: NMT services in modified vehicle to from day program on Tuesdays, Wednesdays, and Fridays. Trip would be from North Mentor to day program on Tyler Blvd. in Mentor and back, around times of 8:30am-2:30pm. Individual or agency provider’s welcome.

DESCRIPTION of INDIVIDUAL: Very intelligent, independent, reasonable, polite, 42-year-old male who has a great sense of humor and loves to make people laugh. Navigates computer with eye gaze, loves Cleveland sports, listens to sports radio, likes country music, and is very independent outside of his physical limitations. He is wheelchair dependent but has no intellectual disability.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Modified Vehicle needed. Individual rides in his wheelchair during transportation.
DIAGNOSES: Quadriplegic. TBI, MS, dystonia, anxiety.
HOUSING: Lives in congregate setting in North Mentor.
DAY PROGRAM: Wants to attend Tuesdays, Wednesdays, and Fridays
SUPPORTS NEEDED: Modified vehicle – May need assistance to operate electric wheelchair upon his request due to spasticity.
