DESCRIPTION OF REQUESTED SERVICES: This individual is 6-year-old male who resides in Willowick. He is looking for HPC services M-F before school from 6:30 AM to 8:30 am and after school from 2:50 PM to 6:30 PM. On weekends, Saturdays from 11 am- 4 PM or 4 PM to- 9 PM and Sundays 12-5 pm. Additional hours are needed on days that he is out of school and while the parent is at work 6:30 am- 6PM. Planned days off of school are:
12/23/24, 12/24/24, 12/26/24, 12/27/24, 12/30/24, 12/31/24, 1/2/25, 1/3/25,1/20/25, 2/17/25, 2/24/25, 3/21/25, 3/24/25 through 3/28/24 (spring break), 4/18/25, 4/21/25, 5/26/25 and 5/30/25 through 8/20/25 (summer break).

DESCRIPTION OF INDIVIDUAL: The individual requires 24/7 supervision for health and safety. He will attempt to elope, bite non-food items such as cords and batteries. This individual has limited verbal skills, and he will use a combination of scripted phrases and will push/pull adults to items he wants or needs.
DIAGNOSES: ADHD, Autism, Developmental language disorder
HOUSING: Individual resides in a townhouse in Willowick with his mother.
DAY PROGRAM/EMPLOYMENT: Elementary School
