DESCRIPTION OF REQUESTED SERVICES: Individual is looking for drop-in HPC services and ATN services in a non-modified vehicle.

SEEKING AGENCY OR INDEPENDENT PROVIDER:  Agency or Independent Providers will be interviewed

FUNDING SOURCE: Level 1 Waiver

DESCRIPTION OF INDIVIDUAL: Individual is a 27-year-old verbal male.  He has no medical or behavioral concerns.  He is very social and enjoys participating in a wide variety of activities.  He desperately wants to make friends and get to spend time with his friends.  He is very visual and responds well to a visual schedule and visual cues to help him complete tasks and organize.  Individual needs verbal reminders to use the restroom if he is involved in an activity as he may wait until it is too late.  He is independent with most ADLs, however, needs water temp set for showers.  He does not tolerate physical assistance with tooth brushing.  He needs verbal assistance to ensure his clothing is on right side out, physical assistance with shirt buttons and tying shoes.  However, he has shoes with laces that do not requiring tying.  He is able to otherwise dress himself.  He is not able to self-administer medications but at this time, only takes a chewable multivitamin and uses Flonase nasal spray, which the family administers.

PROVIDER REQUIREMENTS: Provider will need to deliver all services as identified in the OISP.  Medication Administration Certification is not necessary.

TRANSPORTATION NEEDS: Requesting ATN in a non-modified vehicle in order to attend community activities.

DIAGNOSES:  Moderate ID, ADHD, Anxiety, and PTSD

HOUSING: Lives with family in the Painesville area

DAY PROGRAM/EMPLOYMENT:  Currently seeking full-time ADS/NMT services
