DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC services in the Madison area, up to 15-20 hours/week. Individual drives his own car so will not need ATN services.

DESCRIPTION OF INDIVIDUAL: Individual is a 38-year-old male. He owns his own vehicle, does not have a guardian, and has a significant amount of alone time at home and in the community. He prefers to communicate via text and/or email. He does not participate in group activities, as he is able to travel to visit friends and family as he chooses. He is technologically oriented and having WiFi, his computer and his phone is very important to him.
PROVIDER REQUIREMENTS: He needs physical assistance with financial matters to include budgeting, bill payment, wage reporting, banking and redetermination of benefits (current provider is representative payee and authorized representative and will continue this service until the new provider is established). He also needs verbal assistance to schedule medical, strong encouragement to complete daily hygiene, strong encouragement to complete routine cleaning, strong encouragement to care for his cat, strong encouragement for following his budget, strong encouragement to make healthy lifestyle changes, strong encouragement to complete laundry and wear clean clothing. He will submit his grocery list, and the provider will offer suggestions for healthier options. He is able to self-administer medications and does not need staff certified in Medication Administration. Staff do provide assistance to obtain medications and check for accuracy before delivering them to the individual.

DIAGNOSES: He is diagnosed with Asperger’s Syndrome, Pervasive Developmental Disorder, Mild ID, Acid reflux, High Cholesterol, high Triglycerides, Sleep Apnea, Depression, Anxiety Disorder, Iron Deficiency, PTSD, Obesity, Seizure Disorder, Hx carpal tunnel surgery
HOUSING: In his single apartment in the Madison area
DAY PROGRAM/EMPLOYMENT: He is currently competitively employed in the community.
