DESCRIPTION OF REQUESTED SERVICES: 78-year-old female individual in search of an ADS/Voc Hab setting. Setting will need to be wheelchair accessible. Assistance with daily living skills, restroom/hygiene supports, community engagement, and NMT.

FUNDING SOURCE: IO waiver; would utilize Exit Waiver.

DESCRIPTION OF INDIVIDUAL: Individual utilizes a wheelchair. She can bear some weight, but requires support of a staff member, gait belt, and walker for all transfers. She needs total support for restroom use and is considered a fall risk. Individual experience seizures, but at times will also have “Pseudo Seizures” if she does not feel there is adequate attention given to her. She utilizes a helmet during awake hours to prevent injury if a seizure/fall occur. There are no physical aggression concerns, but staff report that she can verbally escalate when upset with the noise level as she prefers quiet. She is described as a kind individual who enjoys talking about her family. She likes to look at old photos, craft, get her nails done, and watch TV. She really wants to be around peers closer to her age as she feels she cannot connect well with those younger than her. She can move her wheelchair on her own for short distances but requires staff assistance for navigating most areas. She still values work tasks and wants to make a small amount of money.

PROVIDER REQUIREMENTS: Individual has oral and topical medication that she will need assistance with.

TRANSPORTATION NEEDS: Modified Vehicle

DIAGNOSES:  Mild Intellectual Disability, Seizure Disorder, Epilepsy, Depression, Bipolar, Encephalopathy, Osteoporosis, Cerebellar Atrophy, Dementia, Hypertension.

HOUSING: Moving to Madison waiver home- Tentatively 03/24/2025
