DESCRIPTION of INDIVIDUAL: Individual is a joyful, affectionate seven-year-old who enjoys playing with puzzles and toy barns. Although nonverbal, she has a few words and uses adapted ASL. Individual has Cerebral Palsy, which sometimes affects her balance and gives her low muscle tone, making her somewhat clumsy. Additionally, she has uncontrolled epilepsy and a chronic feeding disorder. Individual is seeking someone who will help keep her engaged in activities and caregiver should be comfortable witnessing seizures, administering emergency medication, or calling emergency services while the mother attends to the individual. Her medical appointments are primarily at Nationwide Children’s Hospital in Columbus, requiring the caregiver to be open to traveling with the mother for approximately 2 to 2.5 hours each way (mother will be driving her vehicle, caregiver would be riding with to support and attend to Individual during the ride). Consistency is key for Individual, so it is preferred that one individual take on these responsibilities, as she thrives on a structured routine and familiar settings.
PROVIDER REQUIREMENTS: A provider would need to be Person-directed HPC certified and able to deliver all ISP services as determined at the ISP meeting. Staff will need to be Med Cert 2.
DIAGNOSES: Global Developmental Delay, Mutation of GRIN2B, Quadriplegic Hypotonic Cerebral Palsy, Cortical Visual Impairment, Intractable Epilepsy, Non-verbal, Moderate Encephalopathy
HOUSING: Individual currently lives with her parents in the family home in Mentor.
DAY PROGRAM: Individual is 8 years old and attends online school through Ohio Virtual Academy.
SUPPORTS NEEDED: Assistance with keeping Individual engaged during activities, doctor’s visits, therapy sessions, and providing a respite for her mom in their home in Mentor, Ohio. Be comfortable witnessing seizures, administering emergency medication, or calling emergency services while the mother attends to the Individual. Medical appointments are primarily at Nationwide Children’s Hospital in Columbus, requiring the caregiver to be open to traveling with the Individual and her mother for approximately 2 to 2.5 hours each way (mother drives).
