DESCRIPTION OF REQUESTED SERVICES: The individual is a 17-year-old male who is wheelchair dependent. He lives with his family in Painesville. Currently there is a narrow wooden ramp that is used for his access into/out of the home. It is difficult for his family to negotiate the ramp so he can enter and exit the home. The family would like to work with a provider to explore ramp options that will not take away from their driveway space but will be easier and safer to access to and from the house.
