ADLs: Individual requires verbal reminders to complete personal hygiene tasks thoroughly (bathing, oral care, hair care, deodorant). Assistance completing household chores (dishes, laundry, trash).
Money management: Individual requires assistance managing his finances, budget and benefits.
Medication reminders: Individual requires text reminders to ensuring he takes medications daily.

HOUSING: Individual lives alone in Madison, only requires services in home
DAY PROGRAM/EMPLOYMENT: Competitively employed 5 days weekly
