DESCRIPTION OF INDIVIDUAL: Individual is a 74-year-old male who lives by himself. He prides himself on being a hard worker, he is reliable, polite, and very friendly. He likes to be independent and needs minimal staff support.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: N/A
DIAGNOSES: Autism, high blood pressure and high cholesterol.
HOUSING: Lives by himself in an apartment in Painesville.
DAY PROGRAM/EMPLOYMENT: He works at a job site from September-May, independently.
