DESCRIPTION OF REQUESTED SERVICES: Individual is a 23-year-old male who resides in his family home in Willoughby Hills. He is seeking NMT transportation services on Tuesdays and Thursdays at 2pm, from his work site in Mentor Ohio, to his residence in Willoughby Hills.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Open to either agency or independent provider

DESCRIPTION OF INDIVIDUAL: This individual can be difficult to understand at times, others may need to repeat what he said back to him to make sure they understood him correctly. He does not like it when his routine is not followed, and he should be informed of any upcoming changes in his schedule. He is independent with his mobility. He is reliant on others to be able to access the community. He requires general supervision at home, visual supervision in the community. This individual is fearful of dogs.
PROVIDER REQUIREMENTS: Provider would need to be able to provide services as identified in ISP, in regard to transportation needs.
TRANSPORTATION NEEDS: non modified vehicle.
DIAGNOSES: Autism, Anxiety, Lactose Intolerant
HOUSING: resides in his family home in Willoughby Hills.
DAY PROGRAM/EMPLOYMENT: Individual is employed in a group employment setting in Mentor.
