DESCRIPTION of NEEDED SERVICES: Individual is in need of HPC supports while father is at work. Monday-Saturday 8-5pm. Staff to provide communication and general supervision in the home. Staff would help prepare meals, help with minimal hygiene tasks along with light chores around the house.

DESCRIPTION of INDIVIDUAL: Individual is a 36-year-old male who resides with his father in Mentor.
PROVIDE REQUIREMENTS: This provider will deliver ISP services as documented in the ISP, to a mild-mannered 35-year-old male. Staff will monitor for seizures and follow seizure protocol
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Autism, Severe ID, bipolar disorder, epilepsy, depression, anxiety, verbal and motor tics, impulse control disorder.
Housing: Individual lives in his family home with his father.
