DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an ADS program 5x weekly as well as a NMT provider 10 trips weekly.

DESCRIPTION OF INDIVIDUAL: Individual is a 35-year-old male. He is friendly, good at communicating his wants, he enjoys typing on the computer, eating preferred foods (oranges, orange juice, apples) staying active/walking, pacing, likes putting things away. Assistance with hygiene, visual supervision to monitor seizures. He will ask for staff for high five or a fist bump. Very good swimmer.
PROVIDER REQUIREMENTS: This provider will deliver ISP services as documented in the ISP, to a mild-mannered 35-year-old male. Staff will monitor for seizures and follow seizure protocol
TRANSPORTATION NEEDS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Autism, Severe ID, bipolar disorder, epilepsy, depression, anxiety, verbal and motor tics, impulse control disorder.
HOUSING: Individual lives in his family home in Mentor with his father.
DAY PROGRAM/EMPLOYMENT: Seeking program.
