DESCRIPTION OF REQUESTED SERVICES: Individual is seeking 4-6 hours of HPC/week to assist with community activities/outings. Individual prefers to work with males around his age (30-45). Individual can be reluctant to leave the home until he is familiar with someone. Provider may need to spend time with him in the home to build a rapport before taking him into the community.

DESCRIPTION OF INDIVIDUAL: 33-year-old non-verbal male that communicates using sign language, gestures, facial expressions, and other body language. He can sign up to a 3-word sentence and uses some personalized signs. Individual enjoys going to the Willoughby pool, the mall, movies, swing sets at the park, the beach and riding Laketran. He can be very shy due to his deafness.
PROVIDER REQUIREMENTS: Individual has a Cecostomy tube to assist with bowel movements. Provider will need to be familiar with this device.
TRANSPORTATION NEEDS: Non-modified vehicle. Prefers to ride up front.
DIAGNOSES: Downs Syndrome, deafness with a cochlear implant and mild autism
