DESCRIPTION OF INDIVIDUAL: Individual is a male in his thirties. Individual enjoys going on his computer, watching movies, getting fresh air, and going out in the community. Individual uses a power wheelchair for mobility. Individual has a central line for TPN which is administered at night. Individual has a g-tube.
PROVIDER REQUIREMENTS: Nursing (LPN or RN)
TRANSPORTATION NEEDS: Modified
DIAGNOSES: Individual is diagnosed with the following: cerebral palsy, short bowel syndrome, seizures, osteogenesis imperfecta, acute urinary retention, cellulitis, quadriplegic, exotropia, moderate episodes of recurrent major depression, myopia of both eyes, ADHD, and obstructive sleep apnea
HOUSING: Perry, Ohio.
