DESCRIPTION of INDIVIDUAL: Request is for a very active, sweet natured, sensitive, friendly, fun 39-year-old female that would like to have a break from working 5 days weekly. Looking to attend a day program 1 day weekly (looking at Tuesdays) so she can make new friends, get out in the community, and possibly do some volunteer work. Looking for a positive, friendly environment to talk with peers and have fun with some friends. Loves horseback riding, fishing, sports, eating out, making bracelets to give to others and being with people who give her time to think so she can respond.
PROVIDER REQUIREMENTS: ADS certification. Medications taken at home.
DIAGNOSES: Heart issues, Cognitive Dysfunction, Cerebral Palsy, Mild ID
HOUSING: Lives in Leroy Twp. with her parents.
DAY PROGRAM: Works in the community through group employment, reducing to 4x wk.
SUPPORTS NEEDED: Needs visual supervision while in the community due to being overly friendly. Tends to tell people what she thinks they want to hear rather than her thoughts/feelings and may be quiet when something is wrong. Needs reassurance when in a large crowd or noisy places. Provide verbal assistance for emergencies, reminders to drink water and monitor for dehydration. Verbal assistance with purchases and reminders not to forget/correct change. Verbal reminders when using bathroom.
